Step 4. Mail your application to us

We will not begin processing your application until we have all information we request in our application checklist.

 

It is important that you attach prescriptions, financial documents, and proof of identity.

 

The most common reasons we have to return applications or delayed are because someone:

  • Applied for a type of assistance we do not provide, or for a program that is not open for new applications;

  • Forgot to include a photocopy of ID or other proof of identity;

  • Did not Include any financial documentation to show hardship;

  • Did not include prescriptions or complete information about supplies being ordered (we will not guess on your supplies!).


When you are done, you must mail your application along with any attachments. We do not accept applications via email, phone, or FAX.

NOTE AS OF APRIL 1, 2010 OUR PROGRAMS ARE CLOSED UNTIL FURTHER NOTICE.

Please do not send applications as they will not be acknowledged.

 

Please save your money and do not send registered or certified mail that requires a signature. It will not speed up your application process and we reserve the right to refuse signing for any piece of mail sent to us.
